Co-pays and Deductibles

    All co-pays and deductibles are to be paid at the time of service regardless of who accompanies the child to the appointment. For coverage and deductible related questions please call your insurance provider.

No Call, No Show

    If you are unable to keep a scheduled appointment, please call the office a minimum of 24 hours ahead to cancel or reschedule that appointment. 

Late Arrivals 

If you arrive 15 minutes or more late to your appointment, you will be marked as a no-show and will not be seen, unless you have called ahead and spoken with our front desk staff.

    For sibling appointments, if you arrive 15 minutes late to the first scheduled appointment, we will only be able to see one child, not both.

​School & Camp Forms

    We request you to kindly fill out the parent/student section of the forms so it can be attested by the physician by the end of the visit. There is no additional charge to complete any forms needed for school, camp, sports or other activities provided your child has had a physical exam at our office in the previous year.

Record Transfers

    If transferring to our office from another practice – welcome! Please request your previous doctor’s office to send all medical records to us by fax at (810)584-7512.  

    If transferring out of our office the parent or legal guardian must submit- “Authorization to Release Medical Records” form. We will fax your child’s medical records to his/her new doctor, we do not mail medical records. There is a fee for copying and releasing of medical records directly to patients that is due upon submission of the request. 

​Walk-In Appointments

​    If you need to be seen by one of our providers, please call the office and we will make our best effort to see you in a timely fashion.  Sometimes we are unable to accommodate patients that walk-in the office without an appointment but will assess the situation and give an appropriate appointment.
